It's PAST Time to Renew your Florida License, the Deadline was December 31, 2012.

The two year licensing biennium for Florida Licensed Opticians ends December 31, 2012.  The good news is the licensing fee for renewing the next period has been reduced by $25, down to $125 plus the_$5  unlicensed activity fee, for a total of $130, if you had paid by the deadline.  Since the  first of the year you will need to add an additional $150 to the fee for current total due of $280 for missing the deadline.   

You can still go to www.FLHealthSource.com to renew your licence online or to printout the required form that can be mailed in with your payment.  The Florida Department of Health only sends a post card to all licensees as a renewal notice.  The post card notices where mailed out all opticians in early October their last address of record with the Board office.

Renewal Instructions

  1. Visit http://www.FLHealthsource.com
  2. Click on the “Renew My License” link.
  3. Select the correct profession.
  4. Enter your user name and password.
    Please note your user ID and password were mailed with your physical license.  If you are having trouble accessing your account, please try the alternate login process labeled Get Login Help located at the middle of the login screen.  If you require further assistance, please contact the MQA Comm. Center at 850-488-0595.
  5. Click “Renew License” from the column on the left hand side of the page.

You may RENEW ONLINE if:

  • You do not need to change the status of your license.
  • You have a credit card/debit card to complete the transaction.
  • Your license status is not 'Military, Active'.
  • If you are renewing your license after the expiration date and your status still indicates ‘Clear/Active’ or ‘Clear/Inactive’, you will be assessed a delinquency fee. The delinquency fee will be in addition to your renewal fees.
  • If your license was ‘Delinquent, Active’ or ‘Delinquent, Inactive’ before the renewal deadline, it will be moved to ‘Null and Void’ status after the renewal deadline and you should contact your Board office for instruction.

You may RENEW BY MAIL if:

  • You need to change the status of your license.
  • You need to pay using a cashiers check or money order.
  • You do NOT have a credit card/debit card to complete the transaction.
  • You need to submit your Continuing Education with your renewal (If requested by the Board)
  • Your license is in 'Military, Active' status.
